Prairie Eye Center Ltd., a Springfield-based eye-care provider that operates centers in several local communities, has filed for bankruptcy reorganization.
Prairie Eye listed assets of $5.3 million and liabilities of $10.6 million in its Chapter 11 Bankruptcy, which it filed Dec. 29 in the Central District of Illinois bankruptcy court.
The company operates the Prairie Eye and LASIK Center at 2020 W. Iles Ave. in Springfield as well as the Prairie Retina Center and Prairie Spa at 2800 Montaluma Drive. Prairie Eye also has eye care centers in Jacksonville, Hillsboro and Decatur.
Dr. Sandra Yeh is Prairie Eye’s medical director and is listed in the filing as holding 100% of the company’s stock. Yeh founded Prairie Eye, starting her own practice after the doctor she previously worked with retired, according to the company’s website. She did not respond to an interview request.
Chapter 11 bankruptcy allows companies to restructure their debts while continuing to operate. Prairie Eye previously filed for Chapter 11 bankruptcy in February of 2025, but that filing was terminated in December after a trustee filed a dismissal motion in September, claiming the company had failed to provide proper reports and documents.
In 2023, Prairie Eye reported gross revenue of $14.96 million, according to the bankruptcy filing. That fell to $12.19 million in 2024 and $11.48 million in 2025, for a 23% decline over the three years. The company is projecting gross income of $6.79 million for the first six months of 2026 and total expenses of $6.62 million, for a projected net income of $175,300.
The company listed more than 150 creditors in its bankruptcy filing. The largest creditors are the Bankruptcy Estate of Prairie E&L Management, with a listed trustee in Chapel Hill, North Carolina; CuraScript of Lake Mary, Florida.; and Claris Vision LLC of North Dartmouth, Massachusetts. Both the Prairie E&L Management claim and the Claris Vision claim are listed as disputed claims.
Sumner Bourne of Peoria-based law firm Rafool & Bourne is representing Prairie Eye Center in the case. The case was assigned to Judge Mary Gorman.
Yeh founded her practice in 1996 after Dr Walsh retired not 2020. The W Iles Ave location opened in 1997
